- Discuss opportunities in Phase 1c to provide COVID-19 vaccinations to adults with most common chronic diseases
- Address key concerns about the COVID-19 vaccine among patients who have chronic diseases
- Describe logistics for making COVID-19 vaccination appointments
- Explain available transportation options for COVID-19 vaccination appointments
- Collaboratively identify other barriers and discuss potential solutions
